At CNY Family Care, we believe in building strong, lasting relationships with our patients. To ensure personalized and coordinated care, we encourage each patient to select a personal Primary Care Physician. This physician will be the cornerstone of your healthcare journey at CNY Family Care, overseeing your medical needs and maintaining an ongoing relationship with you. Working in collaboration with a dedicated care team of nurses, Nurse Practitioners, Physician Assistants, and Diabetic Educators, your Primary Care Physician ensures you receive appropriate preventative care, age-related screenings, and the most current, evidence-based treatments for your specific health concerns. We are committed to delivering comprehensive health care tailored to each individual’s unique needs within a family-centered environment.
Accepting New Infant, Adolescent, and Young Adult Patients
Please note that CNY Family Care, LLP does not accept Workers Compensation Insurance. Currently, we are expanding our CNY Family Care family and welcoming new patients who are infants, adolescents, and young adults transitioning from pediatric care. While we are not accepting new adult patients at this time, we encourage you to reach out to learn more about our services for younger family members.
